■公的身分証をお持ちでないお客様はお申込みいただけませんので、忘れずにお持ちください。
■お申込みはお一人様一回のみとなります。
■お名前・ご住所・ご連絡先が一つでも重複している場合、ご応募は無効となります。
■お申込みご本人様のみ有効となります。代理でのお申し込みは無効とさせていただきます。
■ご連絡先が不明瞭の場合、お客様への当選連絡がお知らせできない場合がございますので
_抽選申込書へのお名前・ご住所・ご連絡先のご記入は正確にお願いいたします。
■当選者様のご購入期間は5/3～5/12となっており、ご購入期間内に

read the following application instructions and come to OriginalFake store.

【IMPORTANT NOTICE FOR APPLY】
■These products are lottery sales products applied at OriginalFake store.
■We only accept application over-the-counter at OriginalFake store.
_We do not accept application by phone or internet.
■Customers will need to show their public identification
_(driver’s license, insurance card, passport, etc)
_when signing up for lottery, and also when receiving the product after winning.
■Customers who do not bring their public identification cannot apply for lottery
_so please be sure to bring your public identification.
■Application is allowed only once per customer.
■If either one of your name, your address or your contact information is
_duplicated with another customer, your application will be invalid.
■Application is only effective to the customer who comes to store to sign up.
_Application from deputy will be considered invalid.
■We may not be able to contact the customer for unclear informations so
_please make sure towrite your name, your address and contact information clearly.
■Purchase period for the winner of the lottery is May 3rd to May 12th.
_Customers who can pick up the product during the period can apply
_(We will cancel your order if you cannot come to pickup the product,
_even if you win the lottery).
■Registration is only allowed for one piece per item.
■Even if you apply for multiple items, there may be a case you may not be
_able to purchase all items you applied for due to the order of winning announcement.
_Please note before hand.

【IMPORTANT NOTICE FOR AFTER THE LOTTERY】
■We will only contact the winners by phone after the strict lottery.
■We will not answer inquiries about the lottery results.
■The person who won and applied for the lottery is only allowed to purchase the product.
■Winners will need to show both of thier application receipt and public identification
_(driver’s license, insurance card, passport, etc) when purchasing the product.
■We will decline to sell the product if the winner forgets to bring eather the
_application receipt or their identification.
■Purchase period for winners will be from May 3rd to May 12th.
■If winners cannot come to the store during the purchase period, we will cancel their order.

NOTE: Please note we will not sell or apply for lottery for these items at
ISETAN Shinjuku Men’s Building 2F.
